NHS Continuing Healthcare evidence checklist
This checklist helps families organise information before a Continuing Healthcare assessment. It does not decide eligibility; the NHS assessment process considers the person's individual needs.
Make information easy to follow
Group documents by date and topic. Note who produced each record and keep originals where possible. Focus on evidence of needs and support, rather than trying to predict the outcome.
Ask for current records
Ask the care provider, GP practice and other relevant professionals what information they can provide. Check that the assessment team has the opportunity to consider the most recent evidence.
| Area | Examples to gather | Useful question |
|---|---|---|
| Current care | Care plan, risk assessments, daily notes | What support is needed across a typical day and night? |
| Clinical information | Letters, discharge summaries, medication records | What health needs require skilled oversight? |
| Changes and risks | Dated diary, incident records, falls or distress patterns | How often do needs change or require urgent response? |
| Family perspective | Factual examples of support between professional visits | What is not captured in formal records? |
